Abstract #10.04.02 — The Power of Partnerships: A Multi-Faceted Approach to Address Food Insecurity Author(s) :: S Crimaldi; M Relle; E Grush Author(s) Affiliation(s) :: Ochsner Health Purpose: Region 1 of Louisiana (Orleans, Jefferson, St. Bernard, and Plaquemines) continues to experience challenges with food insecurity. Orleans Parish reports a food insecurity rate of 18.6% (Feeding America), and St. Bernard Parish follows closely at 19.4% (Feeding America), placing them both above the state average of 16.2% (AHR). Addressing this crisis will require a coordinated, data-informed strategy that leverages partnerships to expand access to the Supplemental Nutrition Assistance Program (SNAP), affordable groceries, and food is medicine initiatives. Methods: Ochsner Health took a multi-faceted approach to food insecurity in Region 1 by partnering with government (LDH) and non-profit partners (Second Harvest & Market Umbrella). Our partnership model with LDH places staff at select Ochsner locations to promote SNAP and provide on-site application support. We also teamed up with Second Harvest to expand access to fresh, affordable groceries through the Makin’ Groceries mobile market, and with Market Umbrella to launch a produce-prescription pilot for Medicaid patients with one or more chronic conditions – providing a box of fresh produce delivered directly to their homes. Results/Expected Results: The partnership with LDH is now live at live at 4 sites – Ochsner Medical Center - New Orleans, Ochsner Baptist, and Ochsner Community Health - Gretna and Lapalco. From January 1 and September 30, 2025, LDH staff recorded 303 interactions. The Makin’ Groceries mobile market is operating at 2 locations at Ochsner Medical Center – New Orleans, and served 224 customers between June 24 and November 25, 2025. The produce prescription pilot with Market Umbrella has enrolled 35 patients at the Ochsner Community Health – Brees Family Center. Discussion/Conclusion: By increasing enrollment in SNAP, sustaining the Second Harvest mobile market, and exploring food is medicine initiatives, Ochsner can advance the work on this SDOH and drive meaningful health improvements through coordinated community partnerships.
