Renovascular hypertension is one serious type of blood pressure in which one or both of renal arteries may be narrowed down. This produces a local ischemic state in the kidney and activates the RAS resulting in fluid retention and vasoconstriction. It may present acutely in patients with atherosclerosis as sudden deterioration of control of blood pressure, flash pulmonary edema , hypertensive crisis or deterioration in renal functions on starting the RAS drugs. The fibromuscular dysplasia usually presents as chronic hypertension in young females which is severe and is resistant to treatment. Patients may have associated hypokalemic alkalosis or hyponatremia in some patients. Early recognition and appropriate timely treatment can prevent many complications.
