FUE Hair transplant surgery || Dr. Jayanta Bain Plastic & Cosmetic Surgeon Surgery Technique: Hair transplant surgery involves two stapes, harvesting of graft from the donor area, and then transplanting them over recipient / bald area. Again harvesting can be done by two technique; FUT & FUE. Recipient area marking: This boundary and different zone in recipient area is marked and a rough calculation done about the density of follicle and total number of graft required. Artistic marking for the anterior hair line is very important; it should be made ‘irregularly irregular’ so that it looks natural. Donor area marking: Selection and marking of the donor area should be strictly within the ‘safe zone’ (containing DHT insensitive hair). Anaesthesia: The surgery is done under local anaesthesia, so patient has full consciousness but feel no pain over the scalp. Local anaesthetic medicine is injected around the recipient and donor area, this effect stays 4-5 hours. Punching: It is also known as making holes in the skin for hair graft placement. This is one the most important step of surgery, because the angle of the hole makes the direction of the eruption of the hair. Like, if the angle is made vertical, the hair will erupt straight and look like a string, which looks very unnatural. Follicular Unit Extraction (FUE): A special machine is used which makes 1 mm circular cut around hair follicles, then the hair follicle is extracted by pulling. Advantages: No scar (actually there will be tiny, round, white scars, which are not easily visible). Beard / body hair can only be harvested by this technique only. Graft preservation: After graft harvesting by FUT or by FUE, keeping them alive till implantation is the key for success; thus following steps should be strictly followed: Dehydration: Drying up / dehydration is the most dangerous and most common threat for graft damage. So the grafts should always be kept immerged into graft holding solution. Temperature: Decrease in temperature increases viability of hair follicle; the OT room temperature should be 18-20 deg C & graft holding solution must be 4-6 deg C. Ischemia time: After 4 hours of harvesting, graft starts to desiccate; so less eruption will happen for the grafts staying more time outside of body. Implantation: This is the final step of hair transplant. Grafts should be inserted ‘gently by a single swift stroke’, to avoid any trauma to the follicle.
