The answer to that question is simpler than you thought. Before a medical expert gets on the witness stand, he is prepped by his own attorney extensively. Not only do they discuss the facts of the case, but also how to reply to questions raised on cross examination. This is done by every lawyer and every medical expert who gets on the witness stand. The goal of the attorney who is conducting cross examination is to ask leading questions that either get an expert doctor to agree or disagree with a statement. The goal of a medical expert is not to simply give in and answer limited questions being presented. Instead, the expert's goal is to try and explain at every opportunity why his position is correct. Novice lawyers can sometimes make the mistake of believing they can control a medical expert. They often will run into trouble when the medical expert seeks to explain virtually every answer. The upper hand then goes to the doctor who is making an earnest attempt to educate the jury at that point.
