Laparoscopy is a minimally invasive surgery where we insert small 10 mm and 5 mm ports in the umbilical region and two or three on the lateral side of the abdomen. With the help of a camera, we are able to see all the abdominal organs. Here, the focus is mainly on the pelvic organs; we see the uterus, whether it has any fibroids, and how the ovaries and tubes are. Compared to open surgery, it's definitely a less painful procedure. The size and location of the fibroid also matter. There is something called morcellation, which means that after removing the fibroid, we put it in an endobag, that is, a kind of plastic bag. We place the fibroid inside the bag and morcellate it, i.e., break it into pieces and remove it through the laparoscopic hole we have made. Breaking the fibroid without the bag is not safe because the fibroid can attach elsewhere in the abdominal cavity and start growing. The ideal method is to put it in an endobag, then morcellate and remove it. If it is a feasible fibroid, even up to 8 to 10 cm, we can excise it with an endobag and morcellator. Otherwise, it would be difficult. The size of the fibroid we can remove laparoscopically depends on the instruments and techniques available. Laparoscopic surgery is definitely a minimally invasive surgery, less painful and with quicker recovery time. However, if it is a very large fibroid, even with the morcellator, we may not be able to remove it through the small laparoscopic hole. Typically, fibroids up to 4 to 5 cm can be taken out. If there are multiple fibroids or a scan shows small fibroids scattered all over, we may not be able to remove all of them laparoscopically. In such cases, we opt for open myomectomy to feel and remove as many as possible. The duration of surgery depends on the number and location of fibroids. If it’s a single fibroid in one place, it is less time-consuming. But if there are many fibroids in different places, we have to open at different sites, remove and close each one. That takes longer. Recovery time is faster with laparoscopy. If you are admitted today, by tomorrow or the next day, your pain will reduce, and you can be discharged. Recovery takes about a week; you can resume your routine in 7 to 10 days. Patients often ask what to do and not do post-surgery. We advise avoiding heavy exercise or strenuous work. But since it's a small incision surgery, recovery is quick. When we advise myomectomy, patients often ask whether fibroids will come back. Fibroids are a recurring condition. We remove the visible fibroids, but very small ones (0.5 to 2 cm) might not be visible during surgery. These can grow over time and become visible later on a scan, maybe after 4 to 5 years. Pregnancy is very much possible after fibroid surgery. Depending on the type, subserosal (outside the uterus), intramural (within the wall), or submucosal (inside the cavity), we decide the approach. Subserosal fibroids usually don’t harm pregnancy. Intramural fibroids, if away from the cavity, are also not harmful. Submucosal fibroids may affect implantation, so we advise a rest period of 3 to 4 months post-surgery before trying to conceive. Where you get operated and the technique used matter. Sometimes, adhesions form after surgery. We give medications to prevent these. If taken care of, you can conceive successfully. If fibroids are found incidentally on a scan and you have no symptoms, you don’t need treatment. Only if they are large and causing symptoms like urinary issues (by pressing on the ureters) or constipation (by pressing on the rectum) should they be removed. Otherwise, just monitor them yearly or twice a year. Finding fibroids on a scan alone is not a reason to rush into surgery.
