We see very rarely peptic ulcers and peptic ulcer related disease nowadays after the invention of proton pump inhibitors which are very good drugs to heal duodenal ulcers and gastric ulcers. But peptic ulcer can lead to a complicated scenario which we see as duodenal ulcer perforation or gastric ulcer perforation. This is a surgical emergency because the patient comes with a severe abdominal pain called as peritonitis. Initially it is a chemical peritonitis, later it becomes a gastric peritonitis. We need to intervene it surgically, operate and correct it. The surgical correction is just to close the perforation as early as possible with open surgery or a laparoscopic surgery. So laparoscopic peptic ulcer closure, if the general condition of the patient is better. If it is a very sick patient even the conventional procedure becomes very tough to perform. So laparoscopic surgery always is advantageous to do a simple closure of peptic ulcer closure. So in this case, we do introduce a telescope, do a peritoneal lavage, that is cleansing of the peritoneal cavity with medicated saline and antiseptic solutions and identify the perforated area, suture that and reinforce with the help of omental patch. This is how we do conventionally. Similarly the same technique is carried onto laparoscopic techniques and a perfect closure can be achieved.
