A great confusion exists among clinicians concerning the issue of working length determination. The long heralded concept that modern EALs can be precise only if the instrument reaches the periodontal ligament is meaningless. According to this idea, propagated by many gurus and acritically accepted by most practitioners, the instrument should be first brought to the foramen (ie, beyond the root canal limits, into the periodontal ligament) to be successively withdrawn for an established measure (0.5 - 1 mm). This is patently wrong, for biological reasons (see my Facebook posts on apical limit and patency) and for reasons related to the functioning of EALs. These machines can measure exactly the apical constriction or, in its absence, the point where the istrument is no longer surrounded by mineralized tissue, with NO NEED to go into the PDL first. In this video I am showing a simple routine case in which the working length is established according to the principles of the Biologic Endodontic School.
