An epididymal cyst is a fluid-filled sac located near or behind the testicle, within the epididymis. It is most commonly seen in men aged 20 to 40 and is rare in children. These cysts are usually benign (non-cancerous) and often discovered incidentally during a physical exam or scrotal ultrasound. Small cysts typically require no treatment, just periodic follow-ups every 6–12 months to monitor their size. However, if the cyst becomes large, causes discomfort, pain, or tenderness, medical attention is needed. Treatment may include surgical removal (microsurgical epididymal cyst excision) for symptomatic cases. Temporary options like aspiration or sclerotherapy exist but are less ideal due to recurrence and potential damage to the epididymis. These cysts generally do not affect fertility or cause sexual problems, though a specific type called a spermatocele may occasionally lead to obstruction. Importantly, epididymal cysts are not testicular cancer.
