The Phrygian cap gallbladder is a common anatomical variant where the fundus of the gallbladder partially folds over itself, resembling the ancient Phrygian cap with its conical, forward-leaning top. Typically, this condition is asymptomatic and discovered incidentally during ultrasound imaging. On ultrasound, it presents as a fold or indentation in the gallbladder's fundus, creating an hourglass or bifid shape. The gallbladder wall remains normal, and the fold does not produce acoustic shadowing, which distinguishes it from gallstones. The appearance may change with patient positioning due to the shifting gallbladder contents. Differential diagnoses include gallbladder calculi, septate gallbladder, and other folds like Hartmann's pouch. However, unlike these conditions, the Phrygian cap is benign and does not require treatment.
