In this video, I discuss opioid maintenance treatment, also known as Medication Assisted Treatment (MAT), with buprenorphine (Suboxone et al) and methadone. I address the difference between buprenorphine, a partial agonist, and methadone, a full agonist, and explain the underlying theory of using a long half-life opioid to stabilize / treat addiction to a short half-life opioid. Then, I discuss how the key benefits of these medications carry with them significant risks and drawbacks. Specifically, I touch on: (1) Introduction / differences between buprenorphine (partial agonist) and methadone (full agonist): 0:50 (2) Buprenorphine / methadone help with emotional stabilization BUT can "over-stabilize" / dampen down vitality, creativity, drive: 6:00 (3) Buprenorphine / methadone alleviate opioid withdrawal & associated excitotoxicity BUT come with a host of long-term, significant side effects: 9:02 (4) Buprenorphine / methadone can induce a "remission" in addiction BUT can decrease the impetus to work on underlying mental health, relationship, and other issues: 15:10 (5) Are people on maintenance in recovery?: 20:25 I discuss the side effects of buprenorphine and methadone; the evidence base supporting their use; and the age-old question "Can I be in recovery when I'm on Suboxone / methadone?" #wedorecover #holisticaddictionrecovery #opioidepidemic #opioidaddiction #addictionrecovery #fentanyl #fentanylcrisis #overdose For more science, policy, art, and news related to addiction and recovery, come join us at @concreteconfessional, where I share stories related to being a science teacher / former medical student who has been addicted to benzos and opioids for 15+ years both in the United States and abroad.
