The common belief is that that excessive consumption of phaco energy is always associated with corneal endothelial damage and is the cause for post op corneal edema. In this film i will try to examine this theory . This is an elderly patient who has this hard brown cataract and i use the classic 4 quadrant divide and conquer technique to divide the nucleus . The emulsification of the fragments is performed in a controlled manner at a posterior plane. At the completion of the procedure we see a very high Effective phaco time which is around 6 to 7 times more than my routine cases. The next day evaluation shows a clear cornea with a healthy wound . This case demonstrates that the popular perception that excessive use of phaco energy during a surgery is associated with corneal edema is not entirely true.More important are the following factors 1. The plane at which the energy is delivered 2. The amount of mechanical trauma caused to the endothelium by the flying fragments.
