Dr. Ebraheim’s educational animated video describes the Posterior Cord of the Brachial Plexus in a very easy and simple animation. The posterior cord is made up of the posterior division of all the three trunks, the upper, the middle, and the lower trunk. It usually has fibers from C5 to C8, the presence of T1 fibers is variable. Palsy of the posterior cord is called: Radial Plus Palsy. What is Radial Plus Palsy: You will have radial nerve palsy plus axillary nerve palsy, because they are the terminal two branches of the posterior cord, symptoms include: weakness of the triceps and weakness of the wrist, finger extension will indicate that injury to the radial nerve is located above innervation of the triceps. Injury of the radial nerve occurs probably in the axilla or above, weakness of the wrist, finger extension. There will also be some cutaneous sensory nerve changes where the superficial sensory radial nerve supplies sensation. The axillary nerve injury will cause weakness of abduction due to paralysis of the deltoid muscle, there will also be deficiency in sensation around the shoulder area. Just remember, the axillary nerve also gives innervation to the teres minor muscle. So the radial and axillary nerve are the two terminal branches of the posterior cord of the brachial plexus. What other branches does the posterior cord has? Upper subscapular nerve: spplies the subscapular muscle. Thoracodorsal nerve: supplies the latissimus dorsi muscle, this muscle is very good in flaps of the lower extremity in open fractures, especially the distal third of the tibia, also can be used in the shoulder for a transfer to help young patients with a large rotator cuff tear that cannot be repaired. Lower subscapular nerve: innervates the subscapularis and the teres major muscle. Some people use the word STAR for the branches of the posterior cord. STARS: each letter stands for one of the nerve branches from the posterior cord. Possible exam question: • Somebody have a bullet around the axilla, or somebody may have a brachial plexus injury from a clavicle fracture, it probably will be posterior cord, especially if you have a combination injury of the deltoid, triceps, and weakness in wrist extension.
