This video study presents a unique instance of IOL-bag complex dislocation in a 58-year-old individual with a history of retinal detachment surgery followed by cataract surgery with silicone oil removal. The unconventional nature of the case necessitated an innovative approach to treatment. The patient had undergone retinal detachment surgery involving vitrectomy, band buckle, endolaser, and silicone oil, followed by phacoemulsification, IOL implantation, and silicone oil removal in 2016. Upon presentation in 2024, the patient exhibited dislocation of the entire IOL-bag complex. The surgical intervention involved the removal of the complex via a superotemporal tunnel and the placement of a flapless, glueless, and sutureless scleral-fixated intraocular lens (SFIOL) at an unconventional position due to scarring of ocular tissue at 3 and 9 o’clock resulting from previous buckle placement. The placement of the SFIOL resulted in a stable outcome, and the patient achieved a healthy visual outcome post-surgery. The successful outcome demonstrates that unconventional cases demand innovative solutions, and a thorough understanding of ocular anatomy with a tailor made SFIOL approach can be particularly advantageous in such circumstances.
