Achalasia is a serious condition that affects your esophagus. The lower esophageal sphincter (LES) is a muscular ring that closes off the esophagus from the stomach. If you have achalasia, your LES fails to open up during swallowing, which it’s supposed to do. This leads to a backup of food within your esophagus. This condition can be related to damaged nerves in your esophagus. It can also be due to damage of the LES. Most achalasia treatments involve your LES. Several types of treatment can either temporarily reduce your symptoms or permanently alter the function of the valve. As a first-line therapy, your doctors can either dilate the sphincter or alter it. Pneumatic dilation typically involves inserting a balloon into your esophagus and inflating it. The balloon disrupts the esophageal muscle. The treatment may have to be repeated every few years to ensure long-term results. Another option is Botox to relax the sphincter. Botox is injected into the sphincter through an endoscope.This injection is effective for 3-6 months. A newer option Peroral endoscopic myotomy (POEM) is one of the alternative treatment for achalasia. POEM is a relatively new procedure that uses endoscopic technology and stands for Peroral Endoscopic Myotomy. A benefit of having an endoscopic procedure, there are no incisions in the chest or abdomen and includes a minimal or sometimes no hospital stay post-procedure. A small incision is made in the innermost layer of the esophagus (the mucosa). The endoscope is then tunneled down the remaining length of the esophagus in the layer called the submucosa. In the lower esophagus, the endoscopist cuts the muscle fibers of the lower esophageal sphincter. The endoscope is removed and the first incision made in the mucosa is closed to complete the procedure.
