Whenever a person has got jaundice we do a liver function test the nature of elevation of bilirubin and other enzymes gives us a clue that it could be bile duct obstruction. For an obstructive jaundice like this you do a simple investigation like ultrasound of the abdomen. The ultrasound will show whether there is obstruction or not. There are ducts within the liver that are dilated on ultrasound. It suggests that there is biliary obstruction. The ultrasound also detects if there are any stones in the gallbladder and we suspect the stone might be the cause of obstruction. The ultrasound also shows the level of the obstruction, whether it is above the level of the gall bladder and the cystic duct or is it below that level. When you have an obstruction below the level of cystic duct you usually do a further investigation with CT scan of the abdomen. If you suspect obstruction at the upper abdomen at the level of the liver hilum we usually do MRCP.
