What happens when breast implants develop severe capsular contracture? In this case, I removed saline breast implants along with the surrounding capsules of scar tissue. One implant looked relatively clean, with only a soft, minimal capsule around it. However, the other side told a very different story—it had developed a severe, thickened capsule, leading to contamination, chronic inflammation, and even calcification over time. What is Capsular Contracture? Capsular contracture occurs when the body forms excessive scar tissue around a breast implant. While a thin capsule is a normal part of healing (since the body naturally forms a protective layer around any foreign object), in some cases, the capsule becomes thick, firm, and constrictive, leading to: ✅ Hardening of the breast ✅ Distortion in shape ✅ Pain and discomfort ✅ Inflammation and irritation In this case, the thickened capsule completely encapsulated the implant, causing it to feel firm and unnatural on the chest. The Removal Process During the procedure, I carefully removed both implants along with their capsules. You’ll notice that the side with the severe capsule looks very different—the implant appears contaminated and discolored, likely due to long-term inflammation. When I cut open the capsule, you can see visible calcifications—tiny, rock-hard deposits of calcium buildup, which occur when inflammation persists for a long period. This is a great example of why it’s so important to monitor your implants and see a board-certified plastic surgeon if you experience any changes in the look, feel, or comfort of your implants. Signs You Might Have Capsular Contracture If you have breast implants, it’s important to be aware of these common symptoms that could indicate capsular contracture: ⚠️ Breast feels harder or firmer than normal ⚠️ Implant sits higher than before or looks distorted ⚠️ Pain or discomfort in the breast ⚠️ Noticeable tightness or restriction in movement If you’re experiencing any of these symptoms, don’t wait—consult a board-certified plastic surgeon to discuss your options. In some cases, a capsulectomy (removal of the capsule) and implant replacement may be necessary. This patient had a great outcome after surgery, and we were able to successfully remove the problematic capsule. But this is an important reminder that breast implants are not lifetime devices—they require regular check-ups and monitoring over time.
