This is one of the most commonly performed bariatric procedure at our center. Indications for Sleeve Gastrectomy with proximal jejunal bypass • BMI ≥ 35 with co-morbidities like diabetes, hypertension, sleep apnea, fatty liver, infertility, osteoarthritis knee, Metabolic syndrome or • BMI ≥ 40 without commodities The purpose of sleeve gastrectomy with PJB is to reduce the size of the stomach to the size of a banana. Sleeve PJB is best suitable for volume eaters with type 2 diabetes. Gastric sleeve also results in removal of 90% of hunger hormone i.e. Ghrelin. Sleeve gastrectomy reduces hunger drastically and small stomach causes calorie restriction and weight loss. PJB results in swifter delivery of nutrients in distal ileum resulting in exaggerated incretion hormone response. Sleeve Plus bypass results in weight loss and diabetes remission. Anesthesia Given - General Anaesthesia. Our Technique - We make 5 small incisions (5-10mm) on abdomen. A laparoscope with a light and camera is inserted through 10mm incision, to clearly view the internal organs on a HD monitor. 5mm surgical instruments are inserted through the other incisions to perform the surgery. The first step is to to gastrolysis i.e. separation of the greater omentum fat from the greater curvature of stomach. We do gastrolysis 4cm from the pylorus to Angle of HIS. The second step of the surgery involves gastric stapling. A 36Fr bougie is inserted into the stomach and gastric stapling is done over bougie starting 4cm from pylorus to Angle of HIS. Remnant stomach is removed. We do a dye study to check for a leakage from stapler line. In third step, jejunum is divided 30 cms from ligament of Treitz. 250 cm of small intestine is bypassed and end to side jejuojeunostomy is done.
