The problem with using two hands to hold the capsulorhexis forceps is that you lose the ability to have the other hand assist you. In the case shown here, the patient is simply unable to keep looking at the microscope lights and the eye wanders. This will make it difficult to perform a capsulorhexis. The solution is to use the other hand, the left hand in this case, to fixate the eye by hooking the paracentesis incision with the chopper. Now the right hand can be used to create a well-centered capsulorhexis.
