This is a 35-year old man, seeking treatment for severe spontaneous pain originating from his mandibular second molar. Radiographically there seemed to be only one conical root. Treatment plan encompassed pulpectomy, followed by build-up with composite materials and a metal-free full crown. After acces preparation, a C-shaped canal configuration appeared, with the concavity of the “C” towards the lingual aspect. A mesio-lingual canal could be negotiated, as well as a distal canal. An endodontic instrument inserted in any other segment of the “C” pointed in a distal direction, so that the final morphology was that of a mesio-lingual canal and a “C” connecting the site of the mesio-buccal and the distal canals. The canal system was filled with a premixed bioceramic material. It is interesting how the material, introduced in the mesio-lingual canal, could flow in the distal canal and in the “C”. Build-up was performed with composite materials. No posts were used. The prosthetic preparation was accomplished under magnification, and the finish line checked under operating microscope. Impression with polyeter materials followed. A litium-disilicate crown was prepared. This was cemented under rubber dam isolation.
