The risk of rupture is markedly increased when aneurysm is greater than 2 cm in size. Open surgery or endovascular therapy are both effective and should be performed in these cases because aneurysm rupture occurs with a probability of almost 40%. After rupture the mortality risk can reach the 75%. In the era of endovascular therapies, this option should always be considered as primary strategy of the management for both unruptured and ruptured aneursysms. Successful treatment results in an excellent prognosis with a 5–10 year survival rate of approximately 90%.
