STS Intermacs Database Risk Model for dLVAD Implantation | Annals Animated (April 2025)

In this study, authors developed a 90-day mortality risk model using The Society of Thoracic Surgeons Intermacs registry. Authors queried Intermacs for 11,342 patient cases of primary durable left ventricular assist device (LVAD) implants from January 2019 to September 2023, and using multivariable logistic regression, derived a model based on preimplant risk factors by using derivation (2019-2021 implants) and validation (2022-2023 implants) cohorts. Model performance was then assessed using C-statistics, Brier scores, and calibration plots to create a refined model (all patients) to calculate observed-to-expected (O/E; 95% CI) ratios for each center. The risk model authors created demonstrated satisfactory discrimination and calibration, and is appropriate for informing candidate selection, facilitating quality improvement, and assessing provider performance.
