Viscoelastic Assisted Bubble Formation in Pre Descemet’s Endothelial Keratoplasty (PDEK)

We describe a technical modification of predescemet’s endothelial keratoplasty (PDEK) bubble formation during graft harvesting. In donor graft harvesting, cases in which big bubble (BB) is not formed by initial air injection, viscoelastic can be used to form the BB. With 30G needle attached to the syringe containing viscoelastic 2% hydroxyl propyl methyl cellulose, the needle is advanced into the corneal stroma. Small bubble is initiated and enlarged by controlled injection of viscoelastic into it. The PDEK graft is then dissected and injected into the anterior chamber via an injector. The graft is subsequently unfolded by air or saline and then apposed with the overlying stroma in correct orientation by air. The principle advantages of the technique are easy BB formation when pneumatic dissection has failed and less endothelial injury to the donor graft.
