We just featured a similar complication two days ago in video 1435, so why are we showing another dropping nucleus? Because this is a complication that happens to all cataract surgeons and it is critical that we recognize what leads to this issue so that we can recover. The correct management of this case is the focus of our video today. I encourage you to let a dropping nucleus fall without an effort to chase it and certainly, do not fish for it. The nucleus can be removed by your vitreo-retinal colleagues when the pars plana vitrectomy is performed, usually within a few days to a week or so after your complicated cataract surgery. It is also acceptable to leave the eye aphakic until the vitrectomy/lensectomy is performed, at which time a sulcus IOL can be safely placed without worries about vitreous traction.
