Trichiasis, a very common lid abnormality, is defined as the misdirection of eyelashes toward the eye. Localised distichiasis and trichiasis with resulting keratopathy leads to considerable impairment for the patient. We present a new technique which enables the surgeon to excise the lash with its follicle to prevent regrowth in a single movement. The procedure does not need any preparation, leaving the lid with minimal concomitant trauma which prevents recurrence of trichiasis. The excision is done with a dermatological biopsy punch. The punch is placed on the appropriate place perpendicular to the lid margin where the punch is driven 3-4 mm parallel to the lash. Histopathological diagnosis should be obtained not only to prove the principle of this technique: the entire biopsy material should be handed over to the pathologist as distichiasis and trichiasis can be the first sign of lid tumours. The presented technique of hair follicle excision with a biopsy punch is extremely effective. Success can be proven by the results. (D) Die lokalisierte Distichiasis und Trichiasis mit resultierender Keratopathie verursacht bei den betroffenen Patienten erhebliche Beschwerden. Wir stellen eine Technik vor, welche es dem Chirurgen erlaubt, die Wimper mit dem zugehörigen Haarfollikel in einer einzigen Bewegung zu entfernen, um erneutes Wachstum zu verhindern. Der Eingriff setzt keine Präparation voraus, so dass das Begleittrauma minimiert wird. Die Exzision wird mit einer dermatologischen Biospiestanze ausgeführt. Diese Stanze wird auf der gewünschten Stelle senkrecht zur Lidkante aufgesetzt. Unter Beachtung der Wachstumsrichtung wird die Stanze parallel zur Wimper etwa 3-4 mm vorgetrieben. Es entsteht ein säulenförmiges Exzidat, welches ggf. an der Basis mit einem Scherchen abgeschnitten werden muss. Das gesamte exzidierte Material muss histopathologisch aufgearbeitet werden. Dies soll nicht allein eine Erfolgskontrolle sein: es müssen ebenfalls maligne Tumore ausgeschlossen werden, welche sich nicht selten mit Trichiasis als erstem Symptom ankündigen können. Die vorgestellte Technik der Follikelexzision mit einer Biopsiestanze ist sehr effektiv. Der Erfolg lässt sich klinisch und histologisch nachweisen.
