It can be deadly. While historically associated with battlefields, it is now more common in civilian life following accidents or surgery. Developing gas gangrene requires both clostridial contamination and a significantly lowered oxygen level in the tissue, often due to disrupted blood supply (ischemia/hypoxia) from trauma, vascular damage, or compromised circulation. These bacteria produce potent toxins, including the key alpha toxin, which rapidly damage cells, prevent immune response, cause blood clots, and lead to widespread tissue destruction. Clinical signs include pain disproportionate to the visible injury, swelling, skin colour changes, blisters, and tachycardia. Gas may be felt under the skin (crepitus) or seen on X-ray, but its absence does not rule out the diagnosis. Time is critical for treatment. Management involves a coordinated approach: rapid diagnosis, surgical removal of dead tissue (debridement), aggressive antibiotic therapy (often high-dose Penicillin G), and hyperbaric oxygen therapy (HYPER Hyperbaric oxygen is a crucial adjunct therapy. Evidence shows it improves survival. Mechanisms include stopping alpha toxin production, directly killing bacteria, and helping surgeons distinguish viable tissue during debridement. HBO treatments typically involve breathing 100% oxygen at increased pressure for ~90 minutes per session, often starting before major surgery. HBO is not a standalone cure but significantly reduces morbidity and allows for potentially less extensive surgery when integrated into comprehensive care.
