What Is Haemorrhoid? Haemorrhoids or in layman’s term, we call “piles”. Actually is abnormal engorgement or dilatation of our haemorrhoid veins or blood vessels. It is very common, almost affect 1 in every 2 men. Symptoms Of Haemorrhoid ♦️ Pain ♦️ Bleeding ♦️ Swelling around the anal region What Causes Haemorrhoid? The main reason to cause the veins to dilate is straining. The commonest cause of haemorrhoids now in the modern days is the “Smartphone or Tablet Syndrome". We like to bring our smartphones/tablets to the toilets to do our reading, which will cause us tendency to sit longer in the toilets. With the toilet bowl structure, it will cause a lot of pressure pushing on the blood vessels out and cause haemorrhoids. Internal haemorrhoids means there is a haemorrhoid started from inside the anal or rectum region. When it becomes dilated, it will worsen and grow until we can feel it outside. Symptom wise, internal haemorrhoids usually has no pain but they bleed a lot. External haemorrhoids are haemorrhoids occur outside at the skin near the anal region. It is easily felt and the difference is usually bleeding not that much but it is very painful. Treatment Options for Haemorrhoid ♦️ Lifestyle Modifcation ♦️ Medical Treatments ♦️ Surgical Treatments The first treatment is lifestyle modification. My first advice is do not bring smartphones/tablets into the toilets. Then subsequently, eat more fibers, prevent constipation and do not strain too much. There is Rubber Banding which is very conventional and commonly done. We use a rubber band to tie off the swollen veins (Haemorrhoids). But the failure rate is very high, as high as 40%. Sometimes can cause pain, and infection too. For surgical treatment, there is Conventional Treatments and Minimally Invasive Treatments. In Conventional Treatments or Open Haemorrhoidectomy basically we cut the whole haemorrhoids. So this will leave a large wound. If we over do it (cut too much tissue), we might injure the sphincter muscle which can cause incontinence. Stapler Haemorrhoidectomy where we use a “Stapler”, which made of metal to cut off the whole surrounding 360 degree columns of the haemorrhoids and use that “Stapler” to seal (close) the wound. Subsequently, these foreign materials (metal) will be left behind to maintain the wound. As part of the haemorrhoids structure is cut-off, there is a potential risk of destruction to the surrounding structure. This might give rise to possible complications like bowel incontinence, or formation of strictures if cut too much or cut wrongly. Strictures is narrowing of the anal region (canal) which make passing motion difficult. Incontinence means we lose control in controlling our motion passing. Our faeces (stool) might even come out before we can reach the toilet. Most embarrassing is probably during sleep or during meeting, stool might just come out. Laser Haemorrhoids Treatment is one of the latest treatments. Just make a small cut for the tiny fiber to be inserted into the haemorrhoids. Then there is a light to guide the doctor to move the fiber to the apex (tip) of the haemorrhoids. From there, laser will be activated to seal-off the main source of blood supply. This is done under a controlled manner, with pulses of laser being deliver inside the haemorrhoids to seal off the blood supply and slowly the haemorrhoids will shrink. The advantages of this procedure is there is no cutting, so we do not need to remove any haemorrhoids tissue. As a result, there will be no open wound. The risk of infection is very minimal as well as the pain is also very minimal. Most importantly is that there is no cutting, the incontinence rate so far up to this date is almost zero! Success rate of Laser Haemorrhoids Treatment: Laser Haemorrhoids Treatment up to today research have shown it is comparable to Open Haemorrhoidectomy in terms of success rate. The side effect of the Laser Haemorrhoids Treatment is haemorhoids may take time to shrink because we do not cut. It may take up to 1 to 2 weeks to shrink. Other than that, there is no other major side effects which we are concerned about. In conclusion pain is very minimal after Laser Haemorrhoids Treatment, and most importantly is there is no cutting. The risk of incontinence is almost zero!
