Speaker: Dr. Abhishek Vaidya Topic: Hematoma and injury to major blood vessels during thyroidectomy In this video, Dr. Abhishek talks about: a. How techniques in surgery has improved over the years in thyroids surgery Thyroid surgery has evolved from a highly dangerous procedure with a 40% mortality rate in the mid-19th century to one of the safest and most reproducible surgeries today, thanks to advancements in hemostasis, antisepsis, antibiotics, and refined surgical techniques. While mortality is minimal, the focus has shifted to minimizing morbidity, particularly bleeding complications. b. The risks and dangers posed by hematoma after thyroid surgery Postoperative hematoma is highlighted as the only cause of mortality following thyroid surgery, underscoring the critical importance of meticulous hemostasis during the procedure. Bleeding can also lead to further complications affecting the nerves and parathyroid glands. c. Controlling blood vessels during surgery to minimize complications The speaker details a step-by-step approach to identifying, isolating, and ligating or clipping the superior thyroid artery (emphasizing caution near the external laryngeal nerve), the terminal branches of the inferior thyroid artery (preserving parathyroid supply), and the inconstant thyroid ima artery. d. Strategies and tools used by surgeons to manage bleeding in the arteries and veins of the thyroids Surgeons employ various techniques for hemostasis, including gentle tissue handling, stepwise vessel control, and the appropriate use of energy devices like harmonic scalpels and LigaSure, as well as traditional methods like ligatures and clips. e. How to manage an accidental life-threatening injury to major blood vessels in the neck In the event of a carotid artery injury, immediate digital pressure is paramount, followed by calling for help, obtaining proximal and distal vascular control, and considering repair as ligation carries a high risk of stroke. f. Factors that increase the risk of postoperative bleeding and how surgeons can prevent it The speaker advocates for meticulous intraoperative hemostasis, a warm saline wash at the end of surgery to reveal hidden bleeders, avoiding unnecessary suction on the thyroid bed, and closing the strap muscles loosely to allow any hematoma to escape into the subcutaneous tissue for early detection.
