Brown cataracts are very challenging in terms of their thickness, density and size. Eyes developing brown cataract are likely to have compromised ocular health at the level of cornea and zonules. Doing phacoemulsification in such cases require very controlled fluidic and energy delivery. Inspite of these controlled parameters , the endothelium is always at stake. Manual small incision cataract surgery in eyes with such hard , dense , thick , large brown cataract is a game changer. This video demonstrates steps and outcome of MSICS in brown cataract. Tunnel creation is very critical when it comes to delivery of hard nucleus out of the eye, it should be constructed keeping the density of Nucleus in mind. In the video, two radial cuts are placed at each end of Scleral groove so as to ensure the hassel-free accomodation of the larger thick nucleus.
