This is a patient referred with recurrent optic capture of a scleral sutured CZ70BD. The lens is in good position but despite multiple slit lamp repositionings, placement of 3 laser iridotomies and use Pilocarpine she still has recurrent optic capture. A decision has been made to do a pupil sparing iris cerclage to limit the amount her pupil can dilate so that optic capture is no longer possible. This is done here using a 33 g hypodermic needle sewing machine technique which I recently had accepted for publication in JCRS. The pupil sparing cerclage is done using a running mattress suture 1mm behind the sphincter rather than a running baseball stitch which includes the sphincter and this is so as not to distort the pupil margin. This is a great technique as well for patients with pseudophakic glare that benefit from pilocarpine but cannot tolerate using it every day.
