Once a patient receives a scoliosis diagnosis, they start looking at different treatment approaches, which means they usually figure out fairly quickly that there are two different approaches to scoliosis: traditional treatment and conservative treatment. The traditional approach is more reactive; although it is the number one treatment option, it is really no treatment at all. It is watching and waiting to see if the curve progresses to a point where it becomes severe enough to perform a spinal fusion. The Boston Brace is the most commonly used traditional brace system. The Boston Brace squeezes the spine from the side; this effect can cause some weakness to the muscles and ligaments surrounding the body. It only addresses the spine as a two-dimensional process meaning squeezing from the sides, when scoliosis is a 3-dimensional issue. This type of squeeze can increase postural deviation and rib deformation when you squeeze from the sides; you can push the ribs further back or forward and make the rib deformation more severe. In turn, the more severe the rib deviation becomes, the more it can pull the spine out of alignment. The goal of the Boston Brace is to stop progression, it is not trying to correct the curve, posture, improve function, and compliance is a concern as they are mostly mass-produced to the extent that if two patients are similar circumferences, two different patients can share a brace. They are not made for a specific curve, and they become more and more uncomfortable to wear over time which is the number one sign that the spine is worsening because the brace is not slowing the progression enough, and you are putting the same brace on a bigger curve. There are now many alternatives to Boston braces, including the brace we like to use, the Scoli-Brace system. This brace is a corrective brace; it does not squeeze the spine; instead, it pushes it to provide correction or improvement. Since we are pushing instead of squeezing, it does not create a weakness; instead, it promotes strength. It also addresses the spine in the 3-dimensional manner, dealing with the curve as well as the rotation and compression components associated with scoliosis. Since the end goal is corrective results, each brace is customized to the patient's curvature, and as they improve, the brace becomes easier to wear, which increases compliance and visual results. The more corrective the brace is, the more they need to be managed to try to reduce the curve further. Braces are not the only tool to help reduce the curve; they work adjunct to other types of treatments to complement the other, such as chiropractic care, in-office care, in-office therapy, and rehabilitation; in addition, there are also home exercises that can be prescribed to compliment all the other therapies.
