Major Kirrahe's noble sacrifice comes about if you import a previous save file where Thane did not survive the mission through the Omega-4 relay in Mass Effect 2. If so, Kirrahe, provided you lent him a hand during the Virmire mission in Mass Effect 1, will take Thane's place in saving the Salarian councilor from Kai Leng's murderous hands, at the cost of his life.
