The full educational video link is at the bottom of this decription Patient History: The patient is a 60-year-old female who had all her teeth extracted 30 years ago due to caries. She has been using full upper and lower dentures for the past 30 years, with her lower denture causing discomfort while eating. Medical Risk Factors: No significant medical risk factors. General Diseases: The patient suffers from anxiety, and her psychiatrist supports implant rehabilitation as it significantly impacts her mental health. Medication: The patient takes various medications for her conditions. Anamnesis: The patient struggles to eat with her lower denture due to discomfort caused by pressure on her mental nerves. She desires an implant-supported lower denture for better eating. Intra-oral Findings: The patient currently wears full upper and lower dentures. Severe mandibular resorption with painful mental nerves. The lower denture-to-alveolar crest distance is 17mm. Good oral hygiene is observed. Extra-oral Findings: The patient's occlusion needs adjustment, and the vertical height of her bite should be opened. The original vertical dimension of occlusion is 62mm. Patients' Wish or Complaint: The patient wishes for a lower implant-supported denture to improve her eating experience. General Diagnosis: Removable treatment in the edentulous lower jaw. Radiographic Diagnosis: Good bone quality and density with moderate to severe atrophy at edentulous mandibular sites. No focal osseous abnormality appreciated. Treatment Plan: Due to financial constraints, three implants will be placed in the mandible in selected sites. After a healing period, if stability is optimal, they will be connected with an implant bar supporting equators. The patient will receive a new full upper acrylic denture and a new full lower acrylic implant-supported overdenture connecting to the lower implants via equators on a bar. Surgical Protocol: The patient received intravenous sedation. Local anesthesia was administered. Implant osteotomy sites were prepared using a surgical guide. Healing caps were placed on the implants. Complications: None reported. Prosthodontic Treatment: After implant placement, the patient returned for second-stage procedures eight weeks later. The implants were exposed, and healing caps were placed. Impression copings were fitted, and impressions were taken. The patient's occlusion was adjusted. A post-fitting assessment confirmed a perfect fit. 3-Month Recall Appointment: The patient reported satisfaction and improved eating. Oral soft tissues were healthy, and the patient's oral hygiene was optimal. The video includes in-depth insights and details of the patient's journey to improve her oral health.
