Stage 5, chronic, funnel detachment in an 8 year-old monocular child with a plethora of systemic diseases and possibly the most difficult scenario in all vitreoretinal surgery. The window for functional rescues was probably closed long before the surgery but being a monocular child I wanted to try my best to give the retina its best mechanical chance hoping for a partial reattachment. However due to my best efforts I was unable to reattach what felt like a fibrotic shell and at the 4 weeks followup visit the patient had a closed funnel that I opened using a 360 retinotomy but ended up failing both anatomically and functionally. Hopes and expectations were low but I surely wished I could've done better for this kid.
