These ligaments are broadly classified as true ligaments (fibromuscular) and false ligaments (peritoneal folds). True Ligaments (Fibromuscular Support) 🧬 • Round ligament of uterus – Extends from the uterine cornu through the inguinal canal to the labia majora; derived from the gubernaculum, it helps maintain the uterus in anteversion 🌀 • Uterosacral ligaments – Extend from cervix to sacrum, providing posterior support to the uterus • Cardinal (transverse cervical) ligaments – Anchor the cervix and upper vagina to the lateral pelvic wall; primary support of the uterus, carrying uterine vessels 🚨 • Pubocervical ligaments – Stretch from the pubic bone to the cervix; maintain anterior support False Ligaments (Peritoneal Folds) 🩹 • Broad ligament – A double layer of peritoneum extending from the uterus to the lateral pelvic wall, containing the uterine tube, round ligament, ovarian ligament, and blood vessels • Subdivided into: Mesometrium (supports uterus) Mesosalpinx (encloses fallopian tube) Mesovarium (suspends ovary at hilum) Clinical Correlations: Weakness in these supports leads to uterine prolapse, especially involving cardinal and uterosacral ligaments. During hysterectomy, care must be taken near the cardinal ligament due to close proximity to the ureter ("water under the bridge" 🛑🚰).
