This is a 37 yo lady with microphthalmos, nistagmus, high hyperopia and corneal leucomas with peripheral neovascularisation (on Stem cells deficiency), who came for decrased vision in the last 6-8 months, recently being diagnosed with cataract as well. The video presents the surgery: phacoemulsification, with 35 D PC-IOL implant, followed by in-the-bag implantation of an Artificial iris (AI). The maim challenges in this eye, apart from the risks of a microftalmic eye, were: difficult to perform a precise shape and size of the CCC, due to very elastic anterior capsule and to small anterior chamber, difficulties in implantation in the bag of a thick IOL, and further on, of the artificial iris. The size of the AI cut can be debatable, because the lens bag diameter (assesed by UBM) was much smaler than the white-to-white diameter. Final centration of the AI together with the PCIOL, in such a small bag, were challenging, as well. The limited visualisation, due to corneal leucomas and neovascularisation, throughout the procedure, pose difficulties, too.
