Dr Umang Kothari and his team has pioneered Laparoscopic PPV and has longest follow up of patients. Laparoscopic PPV has its benefits and risk factors but it is a procedure by which a team can deliver as near cis vagina as possible. Peritoneal Pull-Through Vaginoplasty, or Peritoneal Vaginoplasty, is a new transgender surgery solution that's based on a gynecological procedure called the Davydov Procedure. First described in Russian literature, the Davydov Procedure uses abdominal (peritoneal) lining for the creation of the neo-vagina in women born without a vaginal canal. A recent modification was the adaptation of the technique for creating a neo-vagina in transgender women. Peritoneal Vaginoplasty has a number of potential benefits and in the years to come could potentially eclipse other more established procedures, but there's a problem: a lack of long-term data on outcomes for patients who've had either the Davydov Procedure or Peritoneal Vaginoplasty. PROCEDURE DETAILS: During surgery, a laparoscope and several instruments are inserted through small 5-8 mm incisions on the abdomen. These instruments allow dissection of the potential space between the lower urinary tract (urethra, prostatic urethra, and bladder) and rectum. This space will become the future vagina. Peritoneum flaps can be pulled through to the area between the urethra and the rectum to line a portion of the vaginal canal. The top of the vagina is separated from the abdominal contents by closing the peritoneal lining approximately 7-8 inches from the vaginal opening. The remainder of the vaginoplasty procedures (labiaplasty, clitoroplasty, penectomy, orchiectomy, partial urethrectomy, and other associated procedures) are similar to that of the penile inversion technique with some modifications. 1.) Potential benefits of this option: -No hair growth in vagina and pink looking opening of vagina -Self-lubricating lining with some elasticity -Less need for dilation -More vaginal depth and central orientation -Lower risk than colonic vaginoplasty -Anatomical and Physiological as cis vagina as possible 2.) Risks of peritoneal pull-through procedure: -All the risks of the penile inversion vaginoplasty -Additional risk of an abdominal laparoscopic procedure, including intra-abdominal organ injury, ileus, herniation, and others -Flap failure and stenosis -Unknown long-term outcomes In conclusion the new laparoscopic peritoneal pull-through vaginoplasty offers a relatively easy surgical procedure ( in experienced hands ) with excellent results on long term follow up. This procedure is practically devoid of morbidity associated with other techniques. Peritoneal lining having the same parentage of mullerian duct undergoes metaplasia and transforms itself in to stratified squamous epithelium resembling normal vagina.
