this patient was a. known case of hypertension, presented with sudden loss of vision for 1 week. patient had dense pre retina haemorrhage, cause was not clear. patient was advised retina surgery. patient took second opinion where he was injected with anti VEGF. 20 days later patient again came with no improvement of vision. this time b scan revealed reg retina detachment with sub retinal haemorrhagic fluid. surgery was challenging, first view was limited due to excessive bleeding, difficult base shave as blood tinged vitreous was very adherent and third the most difficult part to remove the haemorrhage sub retinal fliuid, this removal of SRF was done by making two retinotomy and pushing the SRF from post pole towards these retinotomies for drainage. complete removal of SRH was not possible. multiple FAE were done to remove the maximum amount of SRF. still the cause is not known for rheg RD, but why anti vegf , what good it will do to the patient with this much of pre retinal haemorrhage. message here in this video is that we should be judicious in using anti veg injections.
