How gastrointestinal ultrasound works is a key topic for clinicians, biomedical engineers and clinical application specialists who want to understand how ultrasound can evaluate the gastrointestinal tract in real clinical practice. In this lecture we explore the fundamental principles of gastrointestinal ultrasound, focusing on wall stratification, sonographic anatomy of the stomach and bowel, and the ultrasound signs that help differentiate Crohn’s disease, inflammatory bowel disease, and gastrointestinal pathology. Starting from the normal layered structure of the gastrointestinal wall, the video explains how the characteristic five-layer ultrasound pattern allows clinicians to recognize abnormalities early. Understanding hyperechoic and hypoechoic layers, mucosa, submucosa, muscularis propria, and serosa is essential for interpreting gastrointestinal ultrasound images. We then analyze the sonographic appearance of several important conditions including gastric carcinoma, Crohn’s disease, bowel wall thickening, inflammatory bowel disease, and diverticulitis. Particular attention is given to the role of Color Doppler ultrasound, wall vascularity, and hyperemia in assessing disease activity. Finally, we discuss how gastrointestinal ultrasound in clinical practice helps clinicians distinguish between inflammatory, obstructive, and neoplastic conditions, supporting faster and more accurate diagnosis at the point of care.
