This was a young lady of 16 years presented with complain of mass upper abdomen, vomiting after taking food. She has history of eating her own hair since childhood. On examination there was a smooth globular mass in upper abdomen. Upper G I Endoscopy done showing a large hair ball, trichobezoar, which was subsequently removed by laparoscopic surgery. Bezoar is collection, concretion of undigested material in gastrointestinal tract. Trichobezoar (hair ball) is formed by ingestion of hair; hair articles.it is usually seen in young adolescent females, who frequently eat their hair. These hairs get accumulated in stomach, small intestine and with period of time become a large ball of hairs. The patient usually presents with symptoms like mass in upper abdomen, fullness in stomach after taking small amount of food, multiple episodes of vomiting, colicky pain in upper abdomen, malaise, weight loss. Usually these patients have some psychiatry disorder so needs psychiatry consultation. Diagnosis usually made by long history if trichophagy, upper abdominal mass, Barium swallow, CT scan abdomen, upper G I endoscopy. Trichobezoar Treatment is mainly surgical -- laparoscopic removal of trichobezoar can be done by expert surgeons, or else it can be removed by open surgical method. Rapunzel syndrome -- it's a rare syndrome where trichobezoar is present in stomach, but hair extension continues into small intestines, may be extended up to terminal ileum. Post-surgical psychiatry consultation is must to prevent recurrence.
