This ia a case of fresh retinal detachment temporally involving the macula with a tear temporally. First of all vitrectomy is carried out to remove all vitreous. After that air fluid exchange is done. Perfluorocarbon heavy liquid is injected to flatten the macula and push the subretinal fluid to the temporal periphery near the pre existing tear. After that endo drainage was done from the tear aspirating with the cutter. Once the retina is flat , endo laser is done all around the tear and is checked by doing peripheral indentation.
