Biliary injuries from cholecystectomy are complex to manage and pose a high risk of postoperative morbidity and mortality. Common postoperative bile duct injuries comprise cyst duct leakage, bile duct injuries, and injuries of the vasculature, particularly the right hepatic artery and the portal vein. There are several known predisposing factors for biliary injuries from cholecystectomy: aberrant, ignored, or misidentified anatomy; complex pathology; excessive hemorrhage; thermal injury; inexperience and/or negligence of the surgeon. The current interventions to manage biliary injuries are varied, ranging from computed tomography (CT)-guided drainage to various endoscopic and surgical techniques. Early recognition and prompt management of a biliary injury are crucial in preventing the potentially resultant morbidity and mortality.
