Source VIMEO SOFT (Trisomy) Conference workshop https://trisomy.org/2024-soft-conference-w... on See also SOFT YT CARE Dr. Mc Caffrey at 32 mins    • Perspectives on the Care and Management of...   um all the cardiac surgeons in the 0:03 country who operate on 0:05 children almost all submit their data to 0:09 the Society of Thoracic Surgeons 0:10 congenital heart surgery database this 0:13 is like 125 hospitals all across North 0:15 America and a group of researchers 0:19 pulled out the data uh from all these 0:21 operations and picked out the ones who 0:24 had a diagnosis of tricomia 18 or 13 for 0:28 this 8-year period including 2010 0:30 through 0:31 2017 and they found that in that 8-year 0:34 interval there were 343 operations uh 0:38 between those two 0:39 diagnoses uh 13 or nine per year uh with 0:44 um uh no I'm sorry um there were nine 0:48 per year 73 patients nine per year with 0:52 tricomia 13 there were 270 patients with 0:55 Tricom 18 that makes 34 patients per 0:58 year um 1:02 it still works out to be a fairly small 1:04 number of operations in any given Center 1:07 this graph is um the percent of centers 1:11 uh so 30% of centers did zero cases in 1:16 eight 1:17 years uh 25% of centers did one case in 1:21 eight years and then fewer centers did 1:26 more uh like seven or eight nine cases 1:30 um and then this final category is 1:32 greater than 10 they're about 7% of 1:34 centers that did more than 10 in an 1:37 eight-year span so basically one a year 1:41 um and 1:44 uh the uh so that really is a small 1:47 number of patients I think I got myself 1:49 out of order I wanted to go to this so 1:52 this is just sort of some back in the 1:54 napin calculation based on the number of 1:58 births that occurred in the US per year 1:59 which is about 4 million the number of 2:02 operations that are reported to the 2:04 database for all children which is about 2:07 25,000 10% of those operations it it has 2:10 been reported in our literature are kids 2:13 with tricomia 2:14 21 looking at triom 218 and 13 what's 2:19 the incidence uh how many babies are 2:21 born with this um and it's lower for 18 2:26 and even lower for 13 of those what 2:29 percent have a heart defect we already 2:31 know those numbers so do the math there 2:34 should be 2150 kids with triom 21 and a 2:38 heart defect there should be 552 kids 2:41 per year in the country with triom 18 2:45 and a heart defect and 101 with Tricom 2:47 13 and a heart 2:48 defect whereas in the database this is 2:53 just eyeballing it but if 10% of kids in 2:55 25,000 cases then there there were 2:58 probably 2500 operations on kids with 3:00 tricomi 3:01 21 and we know from this last study 3:04 there were 34 per year in kids with 3:07 Tricom 18 and nine per year in those 3:10 with Tricom 13 there are more operations 3:13 on on on infants with Down Syndrome than 3:16 there are births with Down Syndrome so 3:19 some of them had more than one operation 3:21 in a year which is expected but I think 3:24 it's safe to assume that fundamentally 3:26 100% of those uh had surgery um um and 3:30 so a center that did 200 children a year 3:35 200 pediatric operations which is a kind 3:37 of a average for you know a two or three 3:40 surgeons kind of your your standard 3:43 issue cardiac center it's not a small 3:45 one it's not a really big one but if a 3:47 center is doing 200 cases a year they're 3:49 probably doing 20 kids with Down 3:53 syndrome and they if you if you just did 3:56 the numbers they should be seeing four 3:59 children 4:00 with a defect and tricomia 18 per year 4:03 so 32 in eight years and they should be 4:06 seeing one with triom 13 and so there 4:10 are a lot of children who have um who 4:15 have defects and who don't get operated 4:18 for a variety of reasons but only 6% of 4:22 the kids born of triome 18 and a defect 4:24 got an operation and 9% with triom 13 so 4:28 very low numbers and some some of that
