According to Abuelela et al. (2012), the triple compression stress test has a sensitivity of 85.9% and a specificity of 100%. It appears that this test has high clinical value for confirming and excluding tarsal tunnel syndrome. However, caution is needed, as only one study has evaluated this test so far. To conduct the test, have your patient lie in a supine position. Maximally plantar flex the patient's ankle, bring the foot and heel into maximal inversion, and compress the posterior tibial nerve posterior to the medial malleolus using your fingers. This compression is applied for 30 seconds. The test is positive if the patient complains of numbness, paresthesia, or pain in the toes, ball of the foot, and/or the heel.
