Free nipple grafts are grafted into place in a more male position during a Double Incision - TOP Surgery. These grafts that make up the nipple areolar complex are bolstered down with sutures and foam padding and a yellow non-stick gauze called Xeroform. After five days these bolsters are removed and the graft “take” or viability is checked. Demonstration of wound care is then taught. This is very simple and is started after the nipple bolsters are removed. It consists of placing a thin coat of triple antibiotic ointment over the wounds and grafts for ten days. After this ten day treatment the patient starts applying scar cream over the wounds. Usually two days after graft bolster removal the patient may shower. I recommend room temperature water with a gentle stream. Using unscented mild soap is best. The patient may wash directly over the incisions and grafts. The shower should be brief and the patient should carefully pat the wounds dry and not rub over the grafts or incisions. A light layer of gauze may be placed over the grafts and incisions to keep ointment off of the surgical garments and clothing. It is important to remove these with great care as the risk of pulling off the graft is possible. A little water over the gauze may help with the removal of particularly “sticky” dressings. The garment may be used to hold the gauze in place as tape can damage the skin. Drains are often left in place after a Double Incision - TOP Surgery. The drains are soft silicone rubber tubes and are left in place to decrease fluid collection in the wound space. The length of time the drains are kept in place varies from individual to individual. The surgeon will decide when the timing is best for drain removal. Placing these drains is the safest way to insure an excellent result and to not compromise the viability of the wound flaps or nipple grafts.
