ANTERIOR CHAMBER INTRAOCULAR LENS IMPLANTATION AFTER PHACOEMULSIFICATION USING SCLERAL TUNNEL INCISION: CASE REPORT Article presented during the 55th ACVO in Savannah, GE - USA on October 24th, 2024, by Dr. Joao Alfredo Kleiner DVM, MSc Purpose: Phacoemulsification in unstable lens can be particularly challenging and sometimes prevents a standard in the bag intraocular lens implantation. This case report shows a technique for anterior chamber intraocular lens (ACIOL) implantation after phacoemulsification using scleral tunnel incision in a 6-year-old Dachshund. Methods: After marking a 6 mm incision on the limbus with a caliper a peritomy to expose the sclera is performed and vessels cauterized. A 6 mm scleral incision 2 mm posterior to the limbus is created with a crescent blade and it is extended to the clear cornea through the stroma without entering the anterior chamber. Three 1.2 mm clear corneal incisions are made for capsule hooks placement and for lens manipulation. A 3.0 mm incision for phacoemulsification is created entering the anterior chamber using the scleral tunnel. After lens emulsification and aspiration, the lens capsule is removed, and an anterior vitrectomy is performed. This stromal incision is enlarged to 6 mm and a rigid 38 diopters, 16 mm total diameter, 6.00 x 7.20 mm optic zone and 1.2 mm vaulting anterior chamber lens is implanted. Sclera is suture with two simple sutures with 9-0 nylon and conjunctiva is burned on the edges with cautery. Results: ACIOL causes minimal inflammation and has a good result in a follow-up of one year. Conclusions: ACIOL implant could be a remarkably interesting option to restore emetropia in dogs.
