3D-CRT in a Breast Case Purpose: Learn how to perform basic 3D-CRT planning on a breast case. Instructions: 1. Import & Open the patient: Breast Patient with Targets. 2. Create a new 3D-CRT plan called “BreastTest”, with 25 fractions and the prescription 5000 cGy to the Median Dose (D50%) of the PTV. 3. Add a beam and an opposing beam with an isocentre in the middle of the PTV. 4. Conform all the beams to the PTV, and then use a 0.5 margin and conform all beams again. 5. Use any of the manual tools to manually edit the MLC leafs. Then UNDO or configure all beams again. 6. Change the gantry angle of beam 1 to 60° and beam 2 to 240° and conform the beams to the PTV again. 7. Calculate the Final Dose and then change the weight of beam 1 to 60%. Introduction to Raystation - Practical Exercises 12 8. Copy beam 1, then use the manual tool to edit the MLC to cover the hot spots on the new beam. 9. Change the MU of the copied beam to 1, calculate dose and then change the weight on the copied beam until the hotspot disappears. 10.Merge the beams. 11.Create a calculation point, place it inside the PTV, and change the prescription to 5000 cGy to the Calculation Point. 12.Use the tools presented to make the best possible breast plan for this patient. 13.Create the Boost beamset, that uses the background dose of the already calculated plan, with 5 fraction and a beamset prescription of 6000 cGy to the Tumor Bed.
