ECG Essentials: Foci vs. Morphology Explained (PVCs & Sick Hearts) Stop using "foci" and "morphology" interchangeably! This quick ECG lesson breaks down the key difference between these two critical terms and explains what they mean for your patient's heart health. Understanding Foci vs. Morphology ✅ Foci or Focus: This term pertains to the area of the heart from where the electrical impulse is originating. ✅ Morphology: This term refers to how the beat looks on the ECG tracing (the shape it makes). PVCs: Unifocal vs. Multifocal We use the example of a Premature Ventricular Contraction (PVC) to show this difference: ▪️A PVC originates from a focal within the ventricles. ▪️A PVC that occurs at one particular focal will take a unique pathway and create a very particular morphology. ▪️Beats originating from the exact same focal will create the exact same morphology. These are called unifocal monomorphic PVCs. ▪️If another area of the heart (another focal) fires off, the shape of the PVC changes because it originated at a different location in the ventricle. This is a polymorphic looking PVC. The Clinical Difference This distinction is more than just academic: multifocal polymorphic beats tend to indicate sicker hearts.
