This video demonstrates rotation and repositioning of a status post YAG laser capsulotomy toric panoptix multifocal intraocular lens (IOL) implant with fixation/securing via reverse optic capture. anterior vitrectomy also performed with triamcinolone staining. This patient had a toric multifocal significantly out of axis/alignment. When the patient did not see well, a previous surgeon performed YAG laser. Apparently only after this was done was it realized how off axis the toric was: instead of the pre-cataract surgery +1.50 of cyl, the patient post cataract surgery then had as much as +3.00 cylinder/astigmatism. Repositioning a lens post YAG is less ideal/more risky/challenging, but it often can still be done successfully. So far (as of time of video production), the patient has only been seen for the POD1 visit, but vision was already significantly better, with the cyl down to 0.50 or less.
