This patient had a small pupil with Pseudoexfoliation which was handled with phacoemusification. After the daunting procedure the trailing haptic of the IOL was broken at the optic haptic junction due to faulty loading technique. IOL explantation was planned. In this case instead of extending the incision to 4.0mm for a bisection or trisection technique which requires proper instrumentation and proper endothelial protection with dispersive viscoelastic.
