There are times when we want to make a small initial capsulorhexis and then enlarge it later to a more appropriate size such as 5 mm in diameter. When we are dealing with an intumescent white cataract, this technique is particularly helpful because we can do the double capsulorhexis method. The first capsulorhexis is small and central with a goal of allowing access to the liquefied lens cortex so that we can decompress the capsular bag. Once we have released this trapped fluid and the intra-lenticular pressure is lower than the pressure in the anterior chamber, we can then enlarge the capsulorhexis to the desired size of 5 mm in diameter.
